Missing a buprenorphine dose happens, and what to do depends mostly on how long it’s been.
A single missed dose: Generally, take it as soon as you remember, unless it’s almost time for the next dose — in which case, skip the missed one and continue on schedule. Buprenorphine’s long duration of action means a single missed daily dose usually doesn’t cause significant withdrawal.
Several missed days: This is where it matters more — after several days without buprenorphine, your tolerance to opioids in general may have started to shift, and restarting isn’t necessarily as simple as resuming your usual dose. What to actually do: Contact your prescriber, rather than guessing, especially if it’s been more than 2–3 days. They may want to reassess your dose or check in about what led to the missed doses, without this being a punitive conversation.
If you’re worried about “getting in trouble” for missing doses: Most addiction medicine providers expect this to happen sometimes and are far more concerned with helping you get back on track than with treating a missed dose as a violation — being honest about it gets you better help than trying to hide it.
